Elder Case Management funded through the Department of Elderly Affairs, the Elder Case Management program is designed to assist Elders and their families with seeking and connecting to services in the community that will allow the Elder to live in the least restrictive setting and as independently as possible for as long as possible.
Personal Choice Waiver A Home and Community Based program designed to assist individuals who meet a Nursing Home level of care and are eligible for Long Term Care Medicaid. The program allows an individual to hire caregivers of their choice to assist them in their home. The age of eligibility is determined as anyone with an adult onset disability or any elder.
(RIPAE) Rhode Island Pharmacetucial Assistance to the Elderly A program designed to assist people 55 and older who are disabled. The program pays a percentage of the cost of certain medicines. It is bease on income and the percent that is paid for the medicines depends on a person’s income. Individuals eligible for Medicare Part D must be enrolled in that program prior to applying for RIPAE
(SHIP) The State Health Insurance Program Assists individuals who are elders or disabled obtain information about insurance options for themselves. Education and support are provided in reviewing options and signing up for assistance with financial programs that provide additional support
(SMP) Senior Medicare Patrol A program designed to assist individuals who have Medicare and/or Medicaid monitor and prevent fraud with their Medicare dollars.
Victims a Crime A program designed to assist Elders who are the victims of crime. Our program guides the person through the police and legal systems and helps them obtain needed services to lessen the impact of the crime.
