Adolescent Self-Sufficiency Collaborative Program
Case management services provided to pregnant and parenting teens who are 20 years of age or younger. The program focuses on school completion and provides participants with on-site 20 hour per week of GED/Life Skills/Parenting Classes. The program ensures that all participants are provided with a holistic case management approach to ensure they are provided with child care, health care, transportation, substance abuse counseling, mental health counseling, affordable housing, basic human needs and other supportive services to ensure teens are transitioned from poverty and into social and economic self-sufficiency. Includes teen fathers, and advocates paternity identification and focusing on male and female responsibility in an attempt to avoid second unwanted pregnancies.

Adult Tobacco Treatment Program
The “Quit for Life” Program provides a smoking cessation program for adults who want to quit smoking. Clients enrolled in the Program are provided with a complete physical assessment through the Agency’s Health Center physicians, and, if eligible, are referred to the Smoking Cessation Program. Clients enrolled, are eligible to receive the nicotine patch, Nicorette gum, Wellbutrin, nasal spray or the inhaler. All clients participating in the clinical aspect of the program must also participate in counseling. The Agency is available to provide this program to private companies and other non-profit organizations.

All Stars Program
Provides services to youth ages 11 to 15 and their families. This is an outcomes-based program designed to prevent substance abuse and violence.

Appliance Management Program
Provides clients with energy conservation audits in their homes to identify heat loss from appliances, light bulbs, water heaters, waterbeds, refrigerators, freezers, clothes washers and dryers, air conditioners, portable space heaters ovens and ranges and lights. The program provides information on smart shopping for appliances; energy education and the installation of electricity saving measures; conservation services to help increase cash payments from participants; walk-through audit in the home with customer participation; and lowers electric bills by approximately 11% saving almost 1,000 kWh each year. Free refrigerators may be awarded to participants in need.

Article 23 Program
Provides comprehensive assessments and case management services to juveniles and their families seeking to file a wayward petition through the Rhode Island Family Court. Program Case Managers act as mediators to help families and their children address issues, which are affecting their emotional, physical, psychological and social well-being.

Basic Human Needs
Provides emergency funds to help pay for miscellaneous items that are not covered under other programs or used when other programs run short of funds. Examples include emergency prescriptions, eyewear, utility shutoff assistance, emergency shelter, emergency transportation, etc.

Boiler Repair
Provides eligible clients with emergency boiler repair services/replacement for heating systems that are beyond repair. The Tri-Town Weatherization Coordinator must obtain special permission from the Rhode Island State Energy Office to replace systems.

Breastfeeding Peer Counseling
Provides information about methods of infant feeding. The ultimate goal is to provide the education and support necessary to increase both the rates and duration of the nursing experience.

Commodities Distribution
Monthly distribution of canned goods, pasta, grain, juice, peanut butter, rice and flour and other foodstuffs as contributed through the State of Rhode Island and as purchased through the Rhode Island Food Bank. All volunteers staff this program.

Elder Case Management Program
Provides senior citizens from the towns of Johnston, North Providence, Smithfield, North Smithfield, Burrillville, Glocester, Scituate, Foster, Cranston, Lincoln, Cumberland and Woonsocket with case management services for elderly in the following categories:

Abuse/neglect; Community and home care; Self-neglect; General assessments; Early intervention
The caseload exceeds 1,400 clients with 11 case managers who hold a minimum of a bachelor’s degree in human services. The program works in close coordination with the Rhode Island Department of Elderly Affairs and the local Senior Centers. Support groups for family members and other caretakers and victims of abuse are provided on a regular basis.

Emergency Housing Assistance Program (EHAP)
This program provides housing counseling and advocacy services through the McKinney Homeless Grant and the Department of Human Services (DHS). . Provides rental/mortgage assistance to those persons in eviction and foreclosure situations and assists with tenant/landlord disputes. Tri-Town works with local building inspectors, housing authority personnel and management companies to secure adequate and affordable housing for the poor.

Even Start Family Literacy Program
This program is funded by the State of Rhode Island Department of Education to provide families with children under the age of 8 with the following services:

GED/ESL/ABE instruction and testing Parent involvement
Literacy skills training Parent Education
Parent and Child activities

The Even Start Program is a partnership with the Smithfield, Johnston, North Providence, North Smithfield, Burrillville, and Foster/Glocester School Departments.

Food Baskets
Seasonal distribution of food baskets to eligible individuals and families. These baskets are the result of donations from the community and local businesses.

Food Vouchers
As needed, vouchers are provided to eligible individuals and families to support their nutritional needs. Funds, provided by the United Way.

Furniture Bank Referral
Tri-Town provides pre-screening and intake for the Rhode Island Furniture Bank for those families in need of assistance in purchasing furniture for their homes. A small fee is charged depending upon availability and items needed.

GED Preparation and Testing
Eligible individuals are provided adult education activities to include academic assessments; group sessions; tutoring; home-based studies; and computer assisted learning.

Head Start Program
The program is designed to provide pregnant women, infants, toddlers and children between the ages of 3-5 years, and their families, with an intensive program designed to provide low-income children and families with a comprehensive program which provides children and families with assistance in reaching their goals of self-sufficiency in a nurturing, healthy and safe environment conducive to learning.

The Tri-Town Head Start Program serves 200 children and families from North Providence, Johnston, Smithfield, North Smithfield, Burrillville, Glocester, Scituate and Foster with part-day and full-day program options. Ten (10) classrooms and three (3) Parent Training Centers are located in Tri-Town buildings in Burrillville, North Providence, and Johnston.

Health Center
No one is ever denied service due to inability to pay. The Health Center accepts third party insurance and RIte Care. A sliding fee scale is available to residents of Johnston, North Providence, Smithfield, North Smithfield, Glocester, and Burrillville, according to income. 24 Hour On-Call Emergency Service is available seven (7) days per week. Hospital Admitting Privileges are provided through Memorial Hospital, Women and Infants Hospital, and Rhode Island Hospital. Tri-Town is a founding member of the Neighborhood Health Plan of Rhode Island (along with the other 13 community health centers statewide). The Health Center is licensed through the Rhode Island Department of Health, is JCAHO Accredited and has been designated as a Federally Qualified Health Center (FQHC), as a Medically Underserved Population (MUP), through the Federal Bureau of Primary Care.

Services provided through the Health Center include:
Pediatrics, Family Resource Counseling, Pre-Natal Care, Health Risk Assessment, Family Medicine, Women’s Cancer Screening Program, Family Planning, Laboratory Testing, Internal Medicine, Diabetes Screening and Management, HIV/AIDS Testing and Counseling, Immunization Clinic, Diabetes Outpatient Education Program, Maternal and Child Health, Diabetes Screening and Management, Pharmaceutical Assistance, Flu Clinic/Pneumonia Vaccines.

Heating/Fuel Assistance
Provides assistance to Rhode Island’s low-income individuals and families, whether they own or rent their homes, with their fuel bills during the winter season. The program generally runs from October through April, with eligible individuals and families receiving a primary grant to help pay their heating bills. Approximately 2,500 individuals and families residing in Johnston, North Providence, Glocester, Smithfield, and Burrillville are assisted annually.

HOME Grant
Provides housing rehabilitation services to eligible families within the community.
Juvenile Hearing Board

The Juvenile Justice Commission funds these three separate Hearing Boards designed to assist juvenile offenders and their families as an alternative to Family Court. Sanctions are imposed, with a case management focus, to assist juvenile delinquents from their second offense while identifying problems affecting their behavior, i.e., truancy, disobedience, delinquency. The Hearing Boards are comprised of members of the community in various fields of expertise including, but not limited to education, welfare, clergy, business, police, etc. The hearing Board services the Towns of Johnston, Smithfield, Scituate, Glocester, and Foster.

Literacy Improvement Program
Provides job training, GED preparation, family literacy, and civic responsibilities classes to low-income at risk individuals in the Tri-Town service area.

Mental Health Counseling and Treatment Program
Provides treatment and counseling services to individuals and families in need of mental health counseling, in individual and group settings. Third party insurance is accepted and no one is denied services due to inability to pay. The Program is staffed by licensed mental health professionals and is JCAHO Accredited.

New Opportunity Homes
Provides adolescent pregnant and parenting teens and their children with a residential independent living program. Funded by the Department of Human Services this facility is located next door to the Tri-Town Community Action Agency Main Building, which increases access to programs directly provided by Tri-Town in their One Stop Setting Site. Participants have the capacity to access provide health care, employment and training, child care, transportation (not needed to come to the main building), WIC, pre-employment and work maturity skills, GED and ESL, Adult Basic Education, Family Literacy, Fuel-Assistance, Weatherization, Emergency Food and Shelter, Self-Sufficiency Case Management Program, Substance Abuse Counseling, Mental Health Counseling, Family Networks, Male Responsibility Program, Adolescent Self-Sufficiency Collaborative, Appliance Management and Parent Education/Parent Aide Program. All participants are provided with on-site supervision during most hours of the day and night through a trained case manager with a minimum bachelor’s degree in social work.

Parent Aide Program
This program, funded by the Smithfield Substance Abuse Task Force, provides parent aide services to eligible families in need of parent aide services. This program is designed to alleviate family stress and to teach appropriate and effective parenting skills.

REACH
The Residential Energy Assistance Challenge Program provides LiHEAP, appliance management program and weatherization clients with energy education and conservation information. In addition, and as appropriate, intensive case management services are provided to eligible clients.

Safe and Drug Free Schools
The Brief Strategic Family Therapy Program, another outcomes based, scientifically proven program provides teens aged 13 – 17 with substance abuse, sexual activity, violence prevention. The program targets at-risk youth, incorporating “protective factors” including anger management, conflict resolution and group counseling in various areas designed to provide youth with critical coping skills needed to succeed in school and in later years.

School-Aged Child Care
Before and after school programs for children through age 12. Fully licensed. Certified teachers. DHS reimbursable. Program is offered in Johnston and North Providence. Full-time Child Care opportunities – offered during the February and April vacations and during the summer.

Shelter
Assistance is provided to eligible individuals and families in need of temporary, emergency shelter.

Substance Abuse Outpatient Counseling/Treatment Center
Individual, group, and family counseling services is provided through this program. Third party insurance is accepted, including Rite Care and NHPRI. Self-Pay, and Sliding Scale Fee is available to all patients. The sliding scale fee is based on income. No one is denied services due to inability to pay. The Substance Abuse Program is staffed by licensed Substance Abuse Counselors and is licensed by the Rhode Island Department of Health. The Program is JCAHO Accredited. Priority is given to Tri-Town clients.

Substance Abuse Prevention Task Force
Tri-Town Community Action Agency coordinates services for the Smithfield and North Providence Task Forces, which fund several substance abuse prevention activities within the towns.

Supportive Employment Program
A collaboration with the RI Department of Human Services to provide job training and job search skills to long-term Welfare recipients. The program is designed to assist individuals and families who have been receiving cash assistance for at least 24 months find career-oriented, unsubsidized employment within two weeks of program participation.

Toy Distribution
Distribution of holiday toys donated by Hasbro to low income residents. Over 4,000 new toys are distributed each year.

Victims of Crime Advocacy Program
Designed to assist all victims of crime over the age of 60 years, with case management, filing of restraining orders, assistance with filing for restitution and/or compensation, assistance with attending pre-trial hearings and follow-up and information, referral and counseling.

Weatherization Assistance
Provides attic/cellar insulation, water pipe wrap, air sealing, caulking, furnace cleaning, efficiency testing of heating systems, and replacement of storm windows and storm doors.

Women, Infants and Children Nutritional Program (WIC)
The Women, Infants, and Children Nutrition Program provides pregnant and post-partum women, infants and children under the age of 5 years with nutritional supplemental programs through monthly food vouchers to purchase nutritious foodstuffs including milk, eggs, cheese, iron fortified cereals, etc. Families are also provided with nutrition education and counseling. WIC services are provided through the Johnston site and a satellite site located in Burrillville.

Youth Responsibility
Serves children and youth who are not pregnant or custodial parents, but are at risk of becoming parents. Both males and females are involved in the program and are provided with a science-based curriculum to assist them in assuming responsibility for their actions and the possible consequences of those actions. Most of the participants are enrolled in public and private school systems and/or are referred to the Agency’s Out of School Youth Program.

Youth Smoking Cessation Program
Smoking cessation groups are offered at the area middle and high schools to assist youth in quitting smoking through funds received from the Rhode Island Department of Health.
