Intensive case management services focusing on school completion and/or - GED; Adult Basic Education and Literacy, ESL, Life Skills Training and Parent Education. Pregnant and parenting teens who are 20 years of age or younger.

All Stars
Substance abuse and juvenile delinquency prevention program. This is an outcomes based, model program which is scientifically based to produce successful outcomes in preventing negative behaviors in teens. At-risk adolescents between the ages of 11 and 16 years old, and their families. Referred through Juvenile Hearing Board, School Departments and through Self-Referral.

Anger Management Groups
12-week clinical treatment program that helps teens find better ways of coping with their anger.  Group will run from 3-4 p.m. in Johnston. The group is open to teen males and females. A pre-group screening is required.  Fees may apply.

Appliance Management Program
Energy conservation audits to identify heat loss from appliances, light bulbs, water heaters, waterbeds, refrigerators, freezers, clothes washers and dryers, air conditioners, portable space heaters ovens and ranges and lights Free refrigerators may be awarded to participants in greatest need. Low income residents of Johnston, North providence, Smithfield, Burrillville, North Smithfield and Glocester.

Article 23
Youth Diversion Program
Provides comprehensive assessments parent education, and case management services for families with children between the ages of 10-17 to address issues, which are affecting the youth and family’s emotional, physical, psychological and social well-being.  Referrals are accepted from Family Court, Juvenile Hearing Boards, Police Depts.,  DCYF, schools, and through self-referral for wayward, disobedient, and/or truant youth from North Providence, Johnston, Smithfield, North Smithfield, Burrillville, Glocester, North Smithfield, Scituate and Foster.

Basic and Early Head Start
Early childhood education, social services, special needs services, child mental health , health and safety, nutrition and assessment, literacy skills training,  parent involvement, parent education, family development, and transition coordination. Pregnant women, infants, toddlers, and children between the ages of 3-5, and their families living in Johnston, N. Providence, Smithfield, N. Smithfield, Burrillville, Glocester, Scituate, and Foster.

Basic Human Needs
Emergency funds to help pay for basic human needs including, heat, electric, health care, prescriptions, etc., when no other financial resources exist. Low income residents of Johnston, North Providence, Smithfield, Burrillville, North Smithfield and Glocester.

Before & After Kindergarten
This program includes lunch and an afternoon snack.  Full-time Child Care opportunities – offered during the February and April vacations and during the summer. Children attending Johnston’s Early Childhood Center program or North Providence Kindergarten.

Behavioral Health Care
(Substance Abuse and Mental Health Treatment and Counseling) Substance Abuse Outpatient Counseling and Treatment, Mental Health Counseling and Treatment Program, Functional Family Therapy; Tobacco Treatment and Counseling (including Nicotine Replacement Therapy). Referrals are accepted on a statewide basis but priority is given to Johnston, North Providence, Smithfield, North Smithfield, Burrillville, and Glocester residents. All third party insurance including NHPRI and Rite Care accepted.  Self-Pay and Sliding Scale Fee is available to all patients.  No one is denied.

Breastfeeding Peer Counseling
Provides information about methods of infant feeding. The ultimate goal is to provide the education and support necessary to increase both the rates and duration of the nursing experience. All WIC eligible clients with pre-weaned children.

Certified Diabetes Outpatient Education (CDOE)
Diabetes Education Program provided in 6 week sessions to patients with diabetes and their caregivers/ significant other. Facilitated by a CDOE Certified Registered Nurse, Dietician, and Pharmacist. Tri-Town is a CDOE Certified site, by the RI Department of Health. Statewide program for individuals diagnosed with diabetes.

Commodities Distribution (Food Bank)
Monthly distribution of canned goods, pasta, grain, juice, peanut butter, rice and flour and other foodstuffs as contributed through the State of Rhode Island and as purchased through the Rhode Island Food Bank. Low income residents of Johnston, North providence, Smithfield, Burrillville, North Smithfield and Glocester.

Community Health Center
A Community Based Health Center (CHC) providing a full range of primary and preventive healthcare services, including family medicine, internal medicine, pediatrics, ob/gyn, laboratory services. Main location in Johnston. No eligibility requirements. Referrals accepted on a statewide basis. All third party insurance accepted.  Sliding scale fee available.

Earned Income Tax Credit
Provides free tax preparation assistance and free electronic filing of State and Federal returns. Low income residents of Johnston, North Providence, Smithfield, Burrillville, North Smithfield and Glocester. Other Towns accepted if appointments available.

Elder Case Management
Case management services for abused/neglected and self-neglected elderly, early intervention, community and home care, general assessments, support groups, caretakers, and victims of abuse, pharmaceutical assistance, and Meals on Wheels.  Seniors ages 55 years and over and adults with disabilities and their families or caregivers residing in the towns of Johnston, N. Providence, Smithfield, N. Smithfield, Burrillville, Glocester, Scituate, Foster, Cranston, Lincoln, Cumberland, and Woonsocket.

Even Start Family Literacy Program

Family Planning Program
Provides CONFIDENTIAL health visits for physicals, PAP smears, STD screening, and other reproductive-related issues. Provides free contraceptives including oral, Depo-Provera, and patches.  Men and woman of reproductive age who are uninsured or underinsure.

Family Resource Counselor
Provides advocacy and assistance with applications for Rite Care and Rite Share.  Provides pre-screening and assistance with Food Stamps, RI Medical Assistance, WIC, Head Start, Fuel Assistance, and Social Services. Fuel Assistance (LIHEAP)

Juvenile Hearing Boards
Offers alternative to Family Court.  Sanctions are imposed, with a case management focus, to divert juvenile offenders from a second offense while identifying problems affecting their behavior, i.e., truancy, disobedience, delinquency. Juvenile offenders and their families in the Towns of Johnston, Smithfield, Scituate, Glocester, and Foster.   Referred directly from local police departments.

Parenting Wisely & Family Functional Therapy
Parenting Wisely is designed to help teens and their families to increase communication; decrease family conflict; increase parental supervision; improve discipline and parenting techniques; and reduce or prevent juvenile delinquency, substance abuse; school failure; violent behaviors, and teen pregnancy. Youth and families referred through Juvenile Hearing Boards in Johnston, Smithfield, Scituate, Foster, Glocester and Burrillville, Truancy Courts, and through Self-Referral.

Personal Choice Waiver
Provides assessment and enrollment to participants; development, implementation, and monitoring of Individual Service and Spending Plan (ISSP); training and guidance to program participants to support self-direction, ensure health and safety through routine program monitoring activities, promote participants independence by providing annual assessments of accessibility, equipment needs, and health education needs; and assistance to client toward oversight of care and arrangement of necessary services.  Referrals are accepted on a statewide basis. Must be eligible for RI Medical Assistance, in need of Personal Care. Only seniors and disabled adults are eligible for services.

Pre-School/Day Care Program
Certified teachers in Early Childhood Education staff the Child Care Center, which is licensed by DCYF. The Program is eligible for tuition reimbursement from DHS. Children between the ages of 3–5 in the towns of Johnston and North Providence.

Prevention Task Force Coalitions
Coordinates the Substance Abuse Prevention Task Force Coalition in each of the three communities (Separate Task Force Coalitions), including all major stakeholders in the community including representatives from schools, housing authority, substance abuse and mental health, faith-based organizations, chambers of commerce, municipalities, senior centers, juvenile justice, youth services, recreation, health care, etc., to establish an annual community-wide plan to deliver substance abuse prevention services in the community. Must be a resident of the Towns of North Providence, Johnston, or Smithfield.

Energy Education and Conservation Program for the elderly including intensive case management services. Statewide. Seniors over the age of 60 and disabled adults who are also eligible for LIHEAP and Weatherization.

Senior Health Insurance Program / RIPAE
Provides assistance with obtaining supplemental Health Insurance, Medicare Part D; and assists with prescription payments through enrolling clients in the RI Pharmaceutical Assistance Program (RIPAE). Statewide referrals accepted. Must be a senior citizen or disabled adult.

Senior Medical Patrol
To coordinate health care fraud, waste and abuse training sessions and presentations in local senior centers, assisted living units, elderly housing units and the community in general; and to assist Medicaid and Medicare beneficiaries to help them navigate the state system. Stanford Model Chronic Disease Self-Management Program (CDSM)
A six week health education program to provide education regarding self-management techniques needed to assist client with chronic disease including diabetes, asthma, depression, cardio-vascular disease, hypertension, cancer. Statewide referrals accepted. Any adult with chronic disease (or their spouse/caregiver), including diabetes, cancer, depression, hypertension, and cardio-vascular disease.

Strategic Prevention Grants
Community Prevention Program for residents of the towns of North Providence and Johnston to conduct a town-wide needs assessment,  capacity building plan, strategic planning, and the implementation of best-practice Substance Abuse Prevention Programs within the schools, community-based organizations and with families. Must be a resident of North Providence or Smithfield.

Victims of Crime Advocacy Program
Case management, filing of restraining orders, assistance with filing for restitution and/or compensation, assistance with attending pre-trial hearings and follow-up and information, referral and counseling.  Statewide for victims of crime over the age of 60 years.

Weatherization and Appliance Management Program
Assists low-income individuals and families to permanently reduce their energy consumptions and increase energy efficiencies. Energy Audits conducted to determine need for insulation, weayjerstripping, pipe sealer, replacement of storm windows and doors. AMP examines electrical use in the home (computerized audit) and replaces light bulbs, small appliances and refrigerators, if they are determined to be inefficient.  Must meet income eligibility guidelines. Serves residents from the Towns of Johnston, North Providence, Smithfield, Burrillville, Glocester.

Women, Infants and Children Nutritional Program
Provides monthly food vouchers to purchase nutritious foodstuffs including milk, eggs, cheese, iron fortified cereals, juice. Monthly Nutrition Education and Counseling services provided by Certified and Licensed Dieticians. Statewide for pregnant and parenting women with infants and children under the age of 5 years. Sites in Johnston and Burrillville.

Women’s Cancer Screening Program
Provides enrollment, screening and treatment for breast and cervical cancer.  Offers free mammograms, PAP testing and office visits for uninsured women. State-Wide program for woman between the ages of 40 to 64.

Youth Responsibility
Science-based curriculum to assist youth in assuming responsibility for their actions and the possible consequences of those actions, includes life skills training, prevention-based curriculum. Non-pregnant or parenting youth who are at risk for teen pregnancy (or second unwanted pregnancy), dropping out of school, substance abuse, and juvenile delinquency.
