For over 35 years, Tri-Town has offered quality, comprehensive health services for the whole family. The Health Center is staffed by Board Certified physicians, nurse practitioners, licensed independent clinical social workers and other health care professionals. Today, in its new, state of the art facility in Johnston, the Center offers medical, dental and behavioral health care all under one roof - and all coordinated and documented through the latest electronic medical records system. All third party insurance is accepted and sliding scale fees are available for those individuals who do not have health insurance and who meet financial eligibility requirements.
Tri-Town Community Action Agency Health Center is in the process of transforming into a Patient Centered Medical Home.
