The Diabetes Education Program, facilitated by a CDOE Certified Registered Nurse, Dietitian, and Pharmacist, provides 6-week sessions to patients with diabetes and their caregivers/significant other. This is a statewide program for individuals diagnosed with diabetes. The Tri-Town Health Center is an American Diabetes Association Recognized Site of Excellence for Diabetes Education and is also a RI Department of Health CDOE Certified Site.

Provides information about methods of infant feeding. The ultimate goal is to provide the education and support necessary to increase both the rates and duration of the nursing experience. All WIC eligible clients with pre-weaned children.

The Health Center monitors and tracks patient health disparities to improve health outcomes and submits the data to the State Registry in the areas of diabetes, pre-diabetes, colo-rectal screening, cardio-vascular and asthma.
