Accessible, affordable medical, dental and behavioral health care for the whole family

For more than 40 years, Tri-County has provided high-quality, accessible, affordable and comprehensive health services for the whole family at our main facility, located in Johnston, Rhode Island. Services provided include preventive and primary health care, mental health and substance abuse treatment and counseling, dental/oral health, and enabling services.

Our new satellite Health Center offers primary and behavioral health services conveniently located on Maple Avenue in North Providence.

Tri-County Health Centers are staffed by board certified and licensed physicians, nurse practitioners, dentists, hygienists, a nurse-midwife, clinical social workers, and other highly qualified and skilled health care professionals.

Good health starts with proper nutrition, and Tri-County Community Action Agency offers several programs to ensure that families have access to nutritious and healthy food, including the Women, Infants, and Children [WIC] nutrition program. Click here to find out more about WIC.

A sliding fee discount is available to all individuals and families, regardless of their ability to pay, and are available to those who meet eligibility requirements based on income and family size and who fall at or below 200 percent of the current federal poverty guidelines.

Tri-County also provides free help with enrollment in healthcare through our Affordable Care Act [ACA] Navigators.

The Tri-County Community Action Agency Health Center is a recognized practice by the National Committee for Quality Assurance [NCQA] as a Level 3 Patient Centered Medical Home [PCMH]. Memberships and Affiliations: 

A participating member of the National Health Service Corps, which offers student loan forgiveness to primary care providers at eligible health centers.
