Tri-Town Community Health Center Expands
In October 2010, Tri-Town completed a $1.5 million dollar renovation project in our Community Health Center, which provided us with and increased capacity to expand access to preventive and primary health care services, pediatrics, obstetrics and gynecology, behavioral health and dental/oral health services. The Health Center now has twelve state of the art exam rooms; electronic medical records; six behavioral health counseling rooms; a patient education, and a health disparities and tobacco treatment and cessation facilitation area. The Health Center is a Certified Diabetes Outpatient Education site recognized by the American Diabetes Association; and with Master Trainers in the Stanford University Model Program: Chronic Disease Self-Management patients are taught how to manage their chronic illnesses. Also, due to a $1.3 million dollar New Access Points Grants funded by the Bureau of Primary Care, Tri-Town was able to offer additional services, including the establishment of a new dental clinic, and hire staff needed to accommodate the increase in patient caseload. The Health Center is staffed by highly qualified, certified and licensed physicians, nurse practitioners, certified nurse mid-wife, dentists, licensed independent clinical social workers; licensed nutritionist and registered dieticians, and registered nurses.
