Requirements for Employment

  • Physical/Certificate of Wellness (within the last year prior to start date).
  • Record of Immunizations (including MMR immunization/immunity, Chicken Pox/Varicella, Hepatitis B, and tDap).
  • TB test and results: All direct care health workers are required to have a 2-step TB test for employment in the Health Center, Dental, Behavioral Health, Adult Day Care, and WIC Departments. If you have had a negative TB test within the last 6 months prior to employment with Tri-Town, and have documentation of that test, the results can be used as your first step TB test. We will then require you to have one additional test prior to your start date. If you work in any other Department within Tri-Town, you will be required to submit a TB test with negative results, that was administered to you within the last 6 months, prior to your start date.
  • tDap Booster or proof of receipt of tDap within the last 10 years.
  • Hepatitis B Series: All employees must provide proof of their Hepatitis B immunization series. Upon hire, if you work in a Department that deems you as significantly at risk for exposure to Hepatitis B, you will be offered the opportunity to receive, at no cost to you, a vaccination for Hepatitis B. If you work in a department that does not consider you as significantly at risk for exposure to Hepatitis B, you will have the opportunity to receive the vaccination, at your own expense, or decline the series at your own will.
  • Influenza Vaccination: Proof of your most recent influenza vaccination must be provided. If you have not received the influenza vaccination, you have the option to receive it through Tri-Town at no cost to you, receive it elsewhere and provide Tri-Town with proof of immunization, or decline the influenza vaccination in writing.

Other:

  • Proof of BCI and/or Fingerprinting. 
  • CANTS (Child Abuse and Neglect Tracking System) Clearance.
  • Criminal Records Affidavit Form.
  • Criminal background check (conducted by Tri-Town) with your written permission.

NOTE:

  •  Depending on the department in which you are hired, it may be necessary to submit additional required information prior to your start date with Tri-Town. Please check with Human Resources regarding specifics.
